IMPORTANT INFORMATION FOR STUDENTS ENTERING THE 6TH GRADE for 2013-14 School Year
ATTENTION
PARENTS OF STUDENTS ENTERING THE 6TH GRADE for 2013-14 School Year:
A dose of tetanus-diphtheria/pertussis (Tdap) vaccine is REQUIRED by the Alabama Department of Public Health for all students aged 11 or older who are entering the 6th GRADE in Alabama. The rule does not require a younger student (e.g., a 10 year old) entering the 6th grade to have the Tdap vaccine. These shots are available at your child’s physician office and the Madison County Health Department. To avoid the last minute rush - PLEASE PLAN EARLY!!!
**If your child’s doctor does not recommend the Tdap vaccine until the age of 12, (according to the law,) your child MUST still have the vaccine so you will need to obtain the Tdap vaccine from another source (ie: the Health Department, or a walk in clinic.)
Please provide an updated Blue Immunization Card, documenting this shot, to YOUR CHILD’S SCHOOL BEFORE school starts.
